Also known as continuing health care or long-term care, Specialist Complex Care is provided to customers with essential, considerable health care needs as a result of chronic illness and/or disabilities. Our aim is to support our customers in leading independent, happy and fulfilled lives. We work in partnership with you and your loved ones to create an all-inclusive care plan that reflects your needs wishes and preferences.
We also work in partnership with other healthcare professionals to ensure safe and smooth transition from hospital to home such as; intensive care specialists, the ventilation team, specialist physiotherapists, dieticians, community matrons and community nurses. Our care workers are DBS checked, comprehensively trained and experienced in delivering specialist complex care interventions.

We have a team of experienced end-of-life care workers available to support both you and your loved ones during these difficult times. Care delivery reflects the NICE Guidance for Care of Dying adults and our end of life care package ensures that we meet customers’ needs and wishes, offering compassion, dignity, and respect.
We provide personal care and emotional support and work with community and specialist services to ensure you feel as comfortable as possible always. Care Plans are developed in partnership with you and your loved ones and will be reviewed as frequently as needed or wanted. Care plans will reflect the everchanging needs and wishes of our customers approaching the end of life

At Excel Care Group our first priority when caring for people with Dementia is to ensure our customers lead a happy, fulfilling life and remain as independent as possible.
